Facial Procedures

The most common operations performed by Dr. McClure are for facial, neck, and eyelid rejuvenation. These are safe operations with predictable outcomes and a fairly quick recovery that leave patients with pleasing natural results. Innovative techniques and new operations are emerging that give today’s patients many more options.

The operations are done as an outpatient in our nationally certified office surgery center typically under local anesthesia with sedation; a safe, soothing, and comfortable experience for our patients. For surgical purposes, we divide the face into three areas corresponding to different operations: the lower face and neck; the upper and lower eyelids; and the forehead and brow. The operations are mixed and matched, depending on the patient’s needs and desires. The overall goals of surgery are to:

  • Refresh the face; that is, to take away a weary fatigued look and help one look less worn out.
  • Rejuvenate the face. Make the face look younger by removing and/or reducing the premature signs of aging.
  • Help patients look less tense, angry, or “grumpy”by correcting inherited muscle, skin, and fat imbalances.
  • Leave patients with a “natural” look, that is, to avoid the unnatural “surgical” look that one often sees in “celebrity” patients.

The Face and Neck Lift by convention is called a “facelift” operation, even though the most dramatic changes are on the neck and jaw line. We tighten the skin and underlying muscle, and remove excess fat beneath the chin to improve the chin/neckline and to contour the jaw. The procedure also improves wrinkles and skin folds of the lower face. Many times adjunct procedures such as Chin AugmentationLipocontouring of the NeckFacial Fat GraftingChemical and Laser skin resurfacing, and the Midface Lift are done to enhance the results.

The LiteLift® is a quicker and more affordable approach to facial rejuvenation for the younger patient who desires a more youthful look but doesn’t require a regular facelift. It is well tested and more reliable than the questionable nationally advertised facial “lifts.”

The upper and lower eyelid procedures, otherwise known as Blepharoplasties, remove excess skin and fat from the lids, giving patients a more refreshed and younger look.

The forehead and brow areas are addressed by the Brow Lift, also known as a forehead lift. This operation elevates the brow to its proper and most aesthetic height, thus relieving heaviness above the eyes and smoothing the crow’s feet area. The frown lines are permanently decreased and the transverse forehead tension lines softened. It is a permanent alternative to BOTOX® Cosmetic  injections.

Rhinoplasty and Otoplasty are common facial operations of the nose and ears for patients of all ages.

Face and Neck Lift

The face and neck are often the first areas we notice signs of aging. As we age we see in the face the loss of skin elasticity, the effects of gravity, damage from exposure to the sun, and the stresses of daily life.  The Face and Neck Lift, restores the chin/neck line, smooths the lower face and eliminates the “jowls.” By convention we call this procedure a “Facelift,” even though a great improvement is seen in the neck. Dr. McClure utilizes numerous techniques and approaches to achieve the desired result based upon the patient’s specific facial anatomy and the amount of skin laxity.  Every patient is different so every operation is a “custom” operation. The ideal facelift, as he performs it, achieves a “refreshed” younger appearance that is natural looking; avoiding the overly tight caught-in-the-wind-tunnel look.

Depending on genetics and environmental factors such as sunlight exposure and nicotine, facial aging can begin in our 30’s, though typically it is first noted in the 40’s or early 50’s. Many younger patients are bothered by signs of aging but have not progressed enough to require a traditional facelift. These patients are often candidates for the Lite-Lift. Many Facelift patients are candidates for complementary or adjunct procedures which can enhance the results. The most common complementary procedures are blepharoplasties, Brow Lift, Chemical Peels (TCA), and BOTOX® Cosmetic or dermal filler injections such as JUVÉDERM® XC

LiteLift® could be called a “mini-facelift.” It is the more affordable option for the younger patient who doesn’t require a traditional facelift. The LiteLift® reduces signs of aging around the neck, jawbone, and lower face. This is a shorter operation with a quicker recovery and longer lasting results which differentiates it from the heavily advertised “lift” procedures marketed nationally. Many patients are rightfully wary of “lift” operations performed by unknown surgeons employed by a national marketing organization. (Many of these surgeons are not board certified plastic surgeons.)

LiteLift® is performed in a safe state of the art operating facility usually under local anesthesia with gentle sedation. Many LiteLift® patients are candidates for complementary or adjunct procedures which can enhance the results.  The most common complementary procedures are blepharoplasties, lipocontouring of the neck, browlift , chemical peels (TCA), and BOTOX® Cosmetic or dermal filler injections such as JUVÉDERM® XC.

The recovery period for the LiteLift® is relatively quick, requiring usually about 10 days for most patients. Adding complementary procedures could lengthen the recovery period depending upon the number of areas treated. Dr. McClure sees patients at least twice before surgery to discuss the operation and all the surgical options.

Neck Liposuction

For the appropriate patient, neck liposuction is a simple and effective procedure. If you have youthful skin elasticity, but are bothered by a double chin or fullness of the neck, you may want to consider this procedure. Through tiny access incisions, a thin liposuction cannula is used to remove excess fat and contour the neck. Recovery is rapid, and the effect can be dramatic. In patients with older skin, the skin may not “snap back” as desired. In such cases, a facelift procedure is more appropriate.

The operation is done under gentle sedation with local anesthesia or local anesthesia alone. It is many times done in conjunction with chin implant, rhinoplasty, Lite lift or facelift.

Brow lifts are designed to elevate the eyebrows to a more youthful position. This “opens up” the eyes and relieves a tired or angry look. It gives one a younger look as well. In females the brow lift can “feminize” the face by giving the brow a natural curve. This procedure also allows weakening of the muscles that create the frown lines such that a permanent Botox-like effect is achieved. By surgically elevating the brows one doesn’t wrinkle the forehead muscles as much, thus smoothing the forehead.

Dr. McClure and Dr. Jackson were the first surgeons in the North Bay to employ the newer endoscopic techniques which enabled the brow lift to be performed through 5 or 6 small incisions placed in the scalp above the hairline; a great advantage over traditional techniques which used a continuous incision across the top of the head. This operation is usually performed under local anesthesia with sedation and may be combined with other eyelid or facial procedures such as the Lite-lift™, a Face and Neck Lift, Chemical Peels (TCA),or Blepharoplasties. The operation is safe and recovery is rapid.

Aging around the eyes produces ezcess upper eyelid skin as well as wrinkles and fat bulges in the lower eyelids, “bags.” One can inherent the tendency for skin and fat excess of the upper and lower eyelids which can make one look worn out or tired when they aren’t. It can make one look older.  Cosmetic eyelid surgery, blepharoplasty, is frequently the most cost effective way of giving one a more refreshed, younger, and less tired look to enhance overall facial appearance.

Upper eyelid surgery involves removing upper eyelid skin excess and fat bulges. The scar falls in the normal eyelid crease and blends in to the point of being almost invisible. Lower eyelid surgery is done by removing excess fat from beneath the eye through an incision made inside of the lower eyelid. This is called the the transconjunctival approach. If there are significant lower eyelid wrinkles the excess skin can be removed through an incision made  just underneath the eyelashes. This scar blends in to the point of being almost invisible. Newer techniques, which combine the internal incisions with laser or chemical resurfacing to reduce wrinkles, are also available. Some patients have lax lower eyelid muscles which require a muscle suspension procedure to obtain the desired effect.

These operations can be combined with other facial procedures such as, Lite-lift, brow lift, chemical peels (TCA), BOTOX® Cosmetic, and Face and Neck Lift.  Blepharoplasties are done under light sedation with local anesthesia. It is a safe operation with a quick recovery.

Midface Lift

Not as common as the facelift or brow lift, the Midface Lift can be an effective procedure for those patients who have experienced drooping of the cheeks with time. In most cases an endoscope is used so that the scars are small and completely hidden. The operation is many times done in conjunction with a brow lift, neck lift, or eyelid procedure.  It is a safe operation with a fairly quick recovery which is typically done under gentle sedation and local anesthesia.

Chemical Peels (TCA)

In addition to the normal aging process, the effects of sun exposure and environmental damage take their toll on our skin, causing it to wrinkle, develop “age” spots and look fatigued. The TCA peel (trichloroacetic acid) is a physician administered deep resurfacing procedure that can be very  effective for:

  • wrinkles
  • uneven pigmentation
  • sun damage
  • blemishes
  • superficial acne scars

A physician administered chemical peel, (TCA peel,) is a much more aggressive peel than the exfoliative type peels done by non physicians. It is not as invasive as laser resurfacing so the recovery time is significantly less. It is often done in conjunction with facial rejuvenation surgery, but also can be performed in the medical spa setting under mild oral sedation. The TCA peel is safe and very well tolerated. There is a fairly pain free recovery period of five to seven days during which time the outer layer of facial skin peels away. Sun protection during the healing phase is very important.

SCITON® Erbium Laser Skin Resurfacing

As we age our skin looses its elasticity and fullness. This aging combined with the effects of sun damage and environmental pollutants takes it toll on the skin leaving it wrinkled, fatigued, and dotted with “age” spots. A Sciton® erbium laser skin resurfacing procedure can reverse these effects and restore a healthy look to the skin.

An erbium YAG laser beam is scanned across the facial areas to remove a layer of skin in a precise manner without bleeding. The body replaces the old, damaged skin with a fresh, healthy layer. Treatments can range from shallow (which might be appropriate for eliminating fine lines, sun spots and tired-looking skin) to deep (which is often used to improve skin tone/texture and eliminate deep wrinkles, even in difficult areas like upper lip lines and crow’s feet).

A variety of skin conditions can be treated including wrinkles, uneven pigmentation, superficial acne scars, “age” spots, and sun damage. The Sciton® erbium laser is considered:

The procedure is performed by our physicians in their nationally certified surgery center, which shares a building with the Medical Spa. Local anesthesia with mild sedation is all that is required for this very safe procedure. We do advise patients that full recovery time can be from two to five weeks depending on the depth of the treatment.

Facial Implants

Youthful facial harmony is dependent on both the skin and the underlying facial skeleton. A well proportioned attractive chin and jawline is key to facial harmony. A receding chin can cause imbalance; making the nose look larger, accentuating jowling, and blunting the neckline. In the appropriate patient, a chin implant can augment the facial bone structure, correcting these shortcomings.

Along the same line, some people have a sunken area beneath the cheekbones, either as a result of their natural bone structure, or as a result of aging. Fat loss in the cheeks result in a sunken, drawn look. Malar (cheek) implants can subtly or dramatically change a patients appearance, depending on the desired result. These implants are not as popular today because we have the new injectable product JUVÉDERM VOLUMA® XC which can augment the cheekbones nicely without the need for surgery and cheek implants.

The chin augmentation procedure is safe and the small scar is well hidden. It is performed under gentle sedation with local anesthesia, and the recovery is quick. Chin implants are many times done to enhance rhinoplasty and facelift results.

Facial Fat Grafting

The technique of Fat grafting  to the face also known as fat injection has improved over the years so that it has now become a common procedure with pleasing and long lasting results. It is done to balance facial proportions and treat facial fat loss which is a sign of aging. Fat grafting can also be used to enhance lip size and correct deep folds on the face. The fat is harvested from the tummy or hip area using special instruments. The harvested fat is refined and purified so that only the fat cells with the best potential for graft survival are used.

The procedure can be done under gentle sedation with local anesthesia, or sometimes under local anesthesia alone. Recovery is quick. In selected patients fat grafting can be used to enhance  breast or buttock size.

Otoplasty is a common operation for children and adults which corrects prominent ears. It is a safe operation with a rapid comfortable recovery. Many times it can be done under local anesthesia alone or with an oral tranquilizer.

The most common ear operation is for the repair of elongated or torn earlobes. This usually occurs with the wearing of heavy earrings or ear gauges. This operation is done under local anesthesia and the earlobes can be re-pierced by Dr. McClure

Rhinoplasty is perhaps the most artistic procedure plastic surgeons perform. Correcting nasal shape or profile may be accomplished with greater precision than ever before. Through careful analysis of nasal and facial proportions, the nose can be modified in order to achieve better balance with other facial features. Dr. McClure spends considerable time before surgery exploring his patients’ desires and preferences with the goal of giving our patients a pleasing aesthetic and natural result.

This is a safe operation performed with gentle sedation and  local anesthesia. Nasal breathing problems may also be addressed at the time of the cosmetic nasal surgery. All incisions are made on the inside of the nose so there are no external scars. The recovery is fairly quick. Many times, chin implant surgery is done at the time of rhinoplasty to help better balance the face.
